your hash file should contain ONLY MD5 hashes, nothing else.

correct hash file format:
aaaaaaaaxxxxxxxxxxxxxxxxxxxxxxxx

incorrect hash file format:
john:aaaaaaaaxxxxxxxxxxxxxxxxxxxxxxxx
